The following Terms of Use is a legal contract between an individual user and Live Oak Health regarding the use of your website and services.
Appointment
If you make an appointment online and do not immediately receive a confirmation email and text, please contact us to confirm your selection.
Confirmations
We must do a confirmation of benefits call to UHC/Optum/UMR, VA, and Medicare to find out what your coverage includes. Kindly call ahead if you'd like to use your insurance.
Cancellations
Your time is important to you and we make every effort to respect that. In return, if you will be late or cannot make it to your appointment, please let us know as soon as possible.
Important: Cancellations with less than 24 hours notice or no-shows are subject to the full appointment charge.
